issue with "Generate Dialog Code"

I'm having an issue with the functionality "Generate Dialog Code..." from
an .ui file. While click on that option the appear an error "There was an
error loading the form */path/of/the/ui/file.ui*". After click *OK* an
empty dialog box appear. If click on "*New...*" button, the error message
shows again.

During Dialog Generate Code:

I'm struck at this point. Can you please suggest me what to do?
